약사 중재에 의해 발견된 항생제 처방 오류 내용 분석

Objective: To analysis frequency and type of antibiotics prescription errors and assess the role of pharmacists in detection and prevention of these errors in this study.
Methods: This study was carried out in antibiotics injection admixture pharmacy in Seoul Asan Medical Center from February to September, 2011. Pharmacists had recorded all antibiotics prescription errors to analyze frequency and type of these errors.
Results: 15,508 antibiotics prescriptions were ordered during 8-month periods. 98 prescription errors(0.63%) of them were detected by pharmacists. There were dilution errors(66.3%), dosing errors(29.6%), other errors(4.1%). In particular, excess of usual dose is approximately 25% in dosing errors.
Conclusion: Pharmacist's intervention is important to reduce the medication errors and prevent potential harms due to prescription errors. But, more studies have to be performed to find a way to reduce the prescription errors.
